The personal information portion of the intake form is available at www.returntohealth.com or can be emailed to you a week before our initial appointment. Please complete this form and bring it with you to our first visit, along with all supplements, medications or vitamins you may be taking. You will next be given several forms that will ask you to record all food and liquids consumed, portion size, and time of consumption. These are tools that we both will use to recognize patterns, problems, and solutions. The remainder of the forms will be explained thoroughly during this first visit.
